PAIN MANAGEMENT PSYCHOLOGY

Though pain is a necessity in physical life (because it warns of harm to the body), chronic pain is a mystery that causes suffering. As a pain psychologist, Dr. Benjamin Hirsch focuses on his patient’s ability to cope with pain, using relaxation techniques, diversion therapy guided imagery and desensitization. A pioneer in the field, Dr. Hirsch is a diplomat of the American Board of Pain Management, a fellow of the American Board of Family Psychologists and an associate of the American Association of Interventional Pain Practitioners. He approaches cases with uniquely designed programs that meet the individual’s specific pain psychology needs, lifestyle and goals.

Some twenty years ago, the field was hardly known. Pain patients who “wouldn’t be healed by medical methods” were considered “crocks”. That was the term given to pain suffers who didn’t get “healed” through the standard medical intervention. There was something “wrong” with them. Practitioners like Dr. Hirsch have helped enlighten the public that there is no such thing as physical pain without a concomitant emotional component.

The sad reality, however, is that so many still do not realize this elementary and obvious truth. They believe that if they do not get better physically, they are somehow responsible. Ironically, that leads to even greater distress. That is sad because many sufferers have learned to live happier and freer lives by using the techniques of psychological pain management.

Physiatry

Physiatry focuses on the diagnosis and treatment of patients who suffer from severe impairments; conditions such as musculoskeletal injuries, headaches or sciatica; and neurological disorders ranging from pinched nerves to carpal tunnel syndrome. Often, a comprehensive approach to dramatic changes in lifestyle is crucial to long- term rehabilitation. Physical Medicine & Rehabilitation physicians may also perform electrodiagnostics which are used to provide nervous system functional information for diagnosis and prognosis for various neuromuscular disorders. The common electrodiagnostic tests performed by physiatrists are nerve conduction studies (NCS) and needle electromyographies (EMG). The nerve conduction study involves electrical stimulation to peripheral nerves, and the nerves’ responses are measured including such things as onset latency, amplitude, and conduction velocity. Needle electromyography requires needle electrode insertion into the muscles to detect the electrical potential generated from muscle fibers. Abnormal electrical potentials, such as fibrillation potential or positive sharp waves, detected by EMG needles indicate the presence of muscle fibers that have abnormal nerve supplies.

Practically any disease condition that is associated with or leaves a person with PAIN, PARALYSIS and/or DEFORMITY, alone or in combination, has a potential to be significantly benefited by physiatric intervention.

PHYSICAL THERAPY

Minimally invasive and non-surgical rehabilitation are more than just buzzwords for suffers of injuries ranging from mild to extreme—they are the only option for successful recovery. Sound musculoskeletal and orthopaedic fitness can be elusive to those who have suffered an injury or lead busy lifestyles. Patients who wish to access world-class care on Long Island can now turn to New York Spine Institute’s licensed physical therapist – Michael Friar.

Physical Therapy is used to encourage mobility and movement, decrease pain, restore function and prevent disability in patients with injuries or impairments.  This is achieved through the use of modalities, active and passive stretching, engaging in an exercise program and manual therapy.  Patients are taught proper body mechanics, postural awareness and home exercise programs to provide maintenance of their current condition and to avoid exacerbations in the future.  The New York Spine Institute offers an array of weight machines to improve strength, in addition to cardio apparatuses for endurance training.  Patients will be evaluated by a licensed physical therapist to determine their current level of function, pain intensity and restrictions with their activities of daily living.  Michael Friar, D.P.T. will discuss the treatment options available to the patient based on the findings from the initial evaluation and set realistic goals in conjunction with the patient to receive the most optimal regimen.

ACUPUNCTURE SERVICES

Acupuncture is the art of inserting thin, disposable, sterile needles at specific locations on the surface of the body. The needles are much finer than the familiar hypodermic needle. Acupuncture accesses the qi or energy of the body and balances it accordingly. Acupuncture has been proven to stimulate the release of endorphins and induce a state of relaxation, balance and healing.

Herbology is the oldest branch of Chinese medicine. It is one of the most effective tools for healing the mind and body. Traditional Chinese herbs are prepared in a balanced formula. The formula is tailored to the individual’s condition and constitution.

Cupping involves the use of glass or bamboo cups placed on areas of the body. The practitioner creates a vacuum by lighting an alcohol swab in the cup and providing suction to the skin. The cups can remain in place or can slide across the body. This is a very pleasant experience with the feeling of a light massage. This therapy removes qi and blood stagnation and promotes normal circulation.

Tui Na is an Oriental Bodywork Therapy that has been used in China for 2,000 years. Methods include the use of hand techniques to massage the soft tissue (muscles and tendons) of the body and acupressure techniques to directly affect the flow of qi.

IMAGING SERVICES

The New York Spine Institute offers state of the art diagnostic imaging: a High Field Ultra Short Bore 1.5T MRI system and a Kodak Point Care Computed Radiography (CR) Digital System.

The high field ultra short bore 1.5T MRI system supports a wide range of clinical applications. Including but not limited to MRI’s of the spine, brain, chest, abdomen, pelvis, shoulder, knee, hip, elbow, wrist, hand, ankle and foot.  Patient comfort is our main priority

The New York Spine Institute Philip 1.5T system provides our physicians with high quality detailed pictures of anatomy and pathology to help them evaluate a wide range of musculoskeletal conditions.

An MRI scans the body for molecular structures using magnetism and radio waves instead of x-rays. Because of the improved computer technology of the 1.5T equipment our patients will experience better and faster exams.

Our system is also equipped with a sound system that will allow our patients to hear their favorite songs while performing the test in a relaxing environment.

This facility uses the Kodak Point of Care computed radiography (CR) in x-ray. In this medium, the captured images are digitized so the radiologist can analyze the soft tissue and bone anatomy for diagnosis.  We also provide Long Length Imaging System (LLI), which lets you capture long-bone CR images for full spine and scoliosis needs.
